Job Description:
*Care Manager MSW at our Little Company of Mary Hospital in Torrance, CA. Positions available are full-time and will work 8-hour Day shifts.
Required Qualifications:
*Master's Degree Social Work.
*1 year 1-3 years recent experience in acute care inpatient setting with patient population in area of service.
*6 months Advanced communication skills to communicate effectively with patients, hospital staff, physicians and the community.
*Computer experience/literacy as needed for the job.
Preferred Qualifications:
*6 months Knowledgeable of community resources with care coordination and discharge planning experience preferred.
